Summary:

The Lake Cumberland District Health Department is accepting applications for one (1) full time HANDS position. Based on experience and qualifications, the position can be either a Family Support Worker I, HANDS Specialist I or HANDS Public Health Nurse I. This position will be based in Taylor Co.


General Duties include, but are not limited to:


Family Support Worker I - Conduct regularly planned visits to establish a trusting rapport for eligible HANDS families; Assist parents with child development, parenting skills, health services and other needed resources identified at the initial assessment or during visits; Maintain confidentiality of program information obtained from home visits and exchanges while communicating with community and support service providers; Provide support to parents during the prenatal period and up to, but not inclusive of the child’s third birthday following the initial parent visit assessment; Facilitate family visitations that may take place in the client’s home or another community site, if justified; Monitor and document the progress of the parent and child(ren); Prepare and maintain case records which shall be documented with contacts, services needed, reports and progress; Confer with the Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers.


HANDS Specialist I and Public Health Nurse I - Conduct parent visit assessment/survey within 30 days of receiving and reviewing prenatal or birth records to determine eligibility for early childhood education and family support services of infants and toddlers from birth up to three (3) years of age. Will conduct interviews to promote outreach activities in family homes or community setting to educate parents in coping techniques and to take an active role in accessing resources or assistance with referrals for appropriate education and nutrition of their child. May develop and distribute informational materials that meet program guidelines. Keep accurate logs of service visits, schedules and charting, prepare reports and analyses set forth in HANDS program policy and guidelines. Interpret laws, regulations, policies and procedures and provide administrative assistance or guidance to departmental staff, which may include but is not limited to; monitor, evaluate and participate in modifications regarding program requirements. Review records and reports submitted by program requests or agency staff and ensure compliance with federal and state law regulation and policies. May conduct ongoing home visitation on an as needed basis.


Minimum Education, Training or Experience:


Family Support Worker I - High School Diploma or GED.


HANDS Specialist I - Bachelor’s Degree in Social Work, Nursing, Early Childhood Education or a Bachelor’s degree in a Social or Behavioral Science related field may also be considered with one (1) year experience performing case management services. Education Substitution: A Master’s degree in Human Services may substitute for the one (1) year of experience.


HANDS Public Health Nurse I - Must have RN license in Kentucky or compact state or provisional license in Kentucky, meeting all requirements of KRS 314.041 and approval to practice as Registered Nurse Applicant (RNA)

Starting Salary / Merit Position Grade: (5% increment added to salary upon successful completion of an initial six months performance probationary period.)

• Family Support Worker I - $13.50 per hour / Grade 10
• HANDS Specialist I - $17.04 per hour / Grade 14
• HANDS Public Health Nurse I - $20.30 / Grade 17

CHARACTERISTICS OF A CLASS : ARE GENERAL STATEMENTS INDICATING THE LEVEL OF RESPONSIBILITY AND DISCRETION OF POSITIONS IN THAT JOB CLASSIFICATION.

• Responsible for initiating and maintaining regular and long term contact and provide support to first time voluntary parents during the prenatal period and extending up to but not inclusive of the child’s second birthday. The entry-level incumbent would participate in on the job training programs to become knowledgeable in introducing parenting skills to empower families to make their own decisions while remaining non-judgmental of families they serve.

• On the job training requirements needed within 6 months of hire in order to advance: HANDS home visitor core training (Must be completed before family contact can occur), HANDS wrap-around required training (Specific topical areas checklist), CPR Certification, Working knowledge of geographic area and or ability to locate participants and community resources with use of maps, contacts, and directions provided.

Provide regular planned visits to establish a trusting relationship for voluntary eligible HANDS participant/s to assist first time parent/s and or families with child development, parenting skills, health services and other needed resources identified at assessment or during visits. Facilitate families in securing of prenatal/child health services, referrals, and appointments with other supportive agencies. Document case records, home visit logs accurately on each contact, progress, service need and completes reports as required. Maintain confidentiality of program information obtained from home visit/s and exchanged while communicating with community and support service providers. Confer with Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers.

TYPICAL WORKING CONDITIONS AND UNIQUE PHYSICAL REQUIREMENTS: INCUMBENTS IN THIS CLASSIFICATION WILL TYPICALLY PERFORM THEIR PRIMARY JOB DUTIES UNDER THESE CONDITIONS. THESE CONDITIONS MAY CHANGE ON OCCASION IN PERFORMING THE DUTIES OF AN INDIVIDUAL POSITION.
